Your First Visit

As a new patient, you will be required to fill out a comprehensive health questionnaire. Completion of the questionnaire begins your health program and the therapeutic process, as well as developing our patient-physician partnership. This enables us to better understand your past and present medical history and all possible conditions.

In addition to the medical questionnaire, you may be requested to bring copies of prior medical records, including lab and x-ray reports.
Dr. Christenson will let you know which documents will be necessary.
